During my last Spearhead game I had a Sherman Bn slugging it out with a PzIV Bn, and coming out on the short end of things. So after the firing phase I decided the Sherman's needed to conduct a Break-Off on the next turn. But my reading of the Break-Off section led me to the conclusion that breaking off would require the Sherman's to turn 180 degrees and allow the Pz IV's a rear shot at them per the Disappearing Target rule, before they could move away.

Does that sound right, or am I misreading something somewhere?

That sounds, yes. The tactical retreat break-off requires two moves (at least) directly away from the enemy force that caused the break-off. Fire attacks, including disappearing target fire, occur after the movement phase.
