I believe the Japanese biological/chemical warfare unit 731 is by far, the single leader in this atrocious category. The current estimates of their victims are between 200,000 and 300,000 Chinese through various bioweapons and an additional 14,000 directly in their facility.

That the Chinese are still very touchy with Japan is, in part, thanks to this horror.
