Oops! But not totally unheard of, I'm aware of similar incidents in Canada, India, the UK and the US to name but four, so they are in good company (at least it wasn't an SSN!)

Well the ship may have had help … e.g. SEALs, etc., or a number of any others nation scuba units.

That sort of incident happens frequently to Third World flagged freighters in Third World ports. I'm not convinced that outside help was necessary in this instance.

It is worth remembering that Iran's two militaries (regular and IRGC) are both relatively poor in terms of actual equipment and personnel quality particularly in terms of technical competence. Add in that the least competent, most religiously fanatic branch, the IRGC, gets first crack at funding and it isn't surprising that things such as helicopter crashes and dismal shipyard outcomes occur on a fairly regular basis.

The beyond dismal performance of Iranian drones and missiles in their essentially zero for three hundred plus attack on Israel should be all the proof needed that the more any nation/service/leader blusters, the results tend to vary in inverse proportion to the volume of hot air.

Very true … like many in that and nearby regions. We have seen their incompetence many times in the past.

Yep, because the IRGC is the Iranian equivalent of the SS who are focused primarily on regime survival and carrying out the dirty deeds of the regime.
