It is entirely possible actions such as this may be needed in the Pacific. It is a regrettable performance and someone should be held responsible if it isn't an unavoidable weather related issue such as Normandy 1944 but more importantly, the entire engineering entities responsible need to learn now, not under near peer fire.

As to holding anyone responsible, even an E-4 or 5, I doubt it. An AH-64 unit deploying to Afghanistan flies its aircraft to Corpus where they are shipped over to meet the pilots in Kuwait. One takes off but the tail wheel stays behind due to a missing pin. No huge problem but it is a delay and represents really sloppy maintenance. Nobody gets disciplined and one AH-64 with two pilots is a heck of a lot more expensive than the pier.

I don't think so even though it appears at times they may not be as Combat Ready as one may think.

Yes, on 19th-20th June at Normandy 1944, The British Mulberry (B) at Arromanches was damaged, the American (A) at Saint-Laurent-sur-Mer was destroyed. Parts from the American pier were used to expedite repairs on (B).

. I don't know if the concept is practical or not. Weather happens. The Mulberries are a good example. Somebody here want to hold Ike accountable? FDR maybe? I think the American Mulberry had been improperly anchored… wasn't this a joint Allied design?

The USN commander of "A" in 1944 became Chief of Staff to the admiral in charge of French coast ports and shipping after the invasion. "A" was not securely anchored to the seabed and could not handle the Channel storm. The damage was too great for repairs. Per Wikipedia.

Actually, like Hobart's 'Funnies' it was a British invention. My (sadly deceased) Dad said, at the time, the failure was due to the Americans taking shortcuts putting it together, but I think in truth the weather conditions are sufficient cause. Even the Arromanches pier barely survived.
The DD tank is another matter, originally designed by a Hungarian, Nicholas Strasussler, it was originally built and modified by British engineers. The failure of the American DD tanks was due to being put into the water too early, and the crews trying too hard to land in their designated landing zone, this maneuver put many of them beam on to the heavy swell, which tragically swamped many of the tanks.
These sad things happen in war, and I salute the bravery of all the participants in D-Day / Operation Neptune.
