Short answer is no, but it happens.

1) He is conflating terms. A misfire is when the firing circuit is complete but the propellant charge does not go off. A hangfire is when the firing circuit is complete but the propellant charge does not go off right away. All the other casualties he describes in the video are not misfires, he never discusses an actual misfire.
2) Hoists are to remain full during firing, the shells in the hoists are what prevents a clear path for any fire to spread down into the projectile flats. Crews parbuckle shells into the hoists using capstans and lines on a greased deck, not always easy on a moving ship with guns firing.
3) In practice, reload times are closer to 45 seconds, not the 30 seconds quoted in the book.
4) At max range, time of flight for the 16"/50 is 89 seconds, if you want to spot your fall of shot and correct you have to wait for the shells to land between salvos.

I did like seeing the records of the engagements though.
