Does anyone know who made this figure and what the name of it was?

Looks like a Grenadier Skellie on a GW horse. |

Thanks definitely a GW horse, the skeleton does look to be in the Grenadier or Ral Partha style. Can't find it in the current Grenadier / Mirliton line, closest is this: link |

And just how does a skeleton blow a horn… with no lungs? |

Thanks the picture I saw of the War Mammoth has the archers wearing helmets. link How does he blow the horn? Magic, same way he walks, fights etc. |

I had thought he had a bow in his other hand, but I am not sure now, no quiver, might be bent spear? |

You were correct Giles I found him on a War mammoth by Grenadier.

Sadly it is OOP, but still around on eBay. Mirliton makes the Mammoth with the catapult.

How can a skeleton blow a horn he has no lips no lungs ??? |

It's like when you put an empty seashell up to your ear to hear the 'ocean'. The horn just amplifies the sound in the skull… ;-) |
