I've seen the HMG mounted to fire forwards or to the rear of the turret. Does the IS-2 cupola rotate or does the gun fire only to the front and rear of the turret? Does the gun manually need to be mounted in the front or rear of the cupola?

The cupola doesn't rotate. It's a mass casting welded onto the turret roof. Judging from these photos:

…and the fact that the commander's split hatch covers can bee seen oriented different directions when opened, suggests some kind of ring mount. Wartime Russian photography doesn't help with a clear view of the actual mount details.
