Laserline Design on Ebay. They used to be Hurlbat and had a web page, but changed their name and are only available from Ebay.
If you don't see what you want send them an ebay message, they're quite amenable. |

IIRC GHQ sells their blades separately to allow you to "update" their older releases. See here; link |
