It has to be Sigismondo Pandolfo Malatesta although he wasn't actually around for the 16th Century Italian Wars.

Federico da Montefeltro is a very a close second for the 15th Century.

For the 16th century, and the "Great Italian Wars" I'm going with Prospero Colonna.
