A damning report! "The LAKE CHAMPLAIN crew was ultimately unprepared for the situation in which they found themselves through a lack of preparation, ineffective command and control, and deficiencies in training." In my experience (30 year USCG) and looking at numerous case studies, there was always someone or someones on the bridge/CIC team, who could have prevented the collision, but for various reasons (junior pers, personality clash, command climate) did not speak up. |
