At least this didn't occur on National Navy Day in India: link Rather embarrassing to have a vessel roll over 90 degrees in port, and a pity about the loss of lives and injuries. Probably didn't do much to improve on the serviceability of the vessel, or morale of the sailors either. |

How does that even happen? Poorly distributed loads, undetected flooding, those kinds of things. Basically, piss-poor crew training and material condition. |

Let's not forget the Indian navy also lost a submarine in 2013 in an explosion at Mumbai (INS Sindhurakshak (S63)). Human error was to blame including fatigue.
And they lost a frigate (INS Vindhyagiri) in 2011 in a collision with a civilian ship.
Three major incidents involving 2 hull losses and another capsizing in 6 years is a pretty woeful record that implies some serious issues in Indian navy capability.
Shades of Santissima Trinidad. Except Santissima Trinidad had been effectively decommissioned since 1989 and used as a spares hulk to keep Hercules afloat (though this has lost its destroyer role and is now a glorified transport ship). Santissima Trinidad has been refloated by the way and is scheduled to become a museum ship dedicated to Falklands War. INS Betwa was a fully functional warship only commissioned in 2004.
