Is the Old Glory Command Decision CD-134 M4A4 Sherman the Sherman V?

I thought the A4 was primary a Lend-Lease version along with the A2 Diesel which was also used by the USMC in the Pacific.

Yes, the M4A4 Medium is the Sherman Mk V.

M4 = Sherman I
M4A1 = Sherman II
M4A2 = Sherman III
M4A3 = Sherman IV (not used by Commonwealth)
M4A4 = Sherman V

Other common variants:

Sherman Ib (M4 105mm)
Sherman I Hybrid (M4 hull with cast glacis plate added)
Sherman Ic Hybrid 'Firefly' (as above with 17pdr)
Sherman IIa (M4A1 76mm W)
Sherman Vc 'Firefly' (Sherman V with 17pdr)
Sherman V 'Crab' Flail Tank

Bonjour Jemima Fawr,

Your Sherman "summary" is very helpful as although I'm not entirely dim, I find Sherman model types somewhat perplexing.

In this I believe that I'm in good company as Winston Churchill struggled with this as well.

I just wondered if there is an "Idiots Guide" which illustrates the various Sherman types with ideally both line-drawings and pictures, and, if there is whether you or some other kind person can tell me where to acquire it?
