romea.cz has a picture of the leader of the self-proclaimed, unrecognized Donetsk People's Republic, Denis Pushilin, awarding military decorations to a soldier. The soldier wears a death's-head and a patch with 3 conjoined triangle. Is this a unit insignia, neo-nazi symbol, or other?

I suppose that a Swedish lumber company could be neo-Nazi …

From Wikipedia:

The symbol is used for a variety of purposes in modern popular culture. The valknut symbol is used as a religious symbol by adherents of Heathenry, a new religious movement inspired by historic Germanic paganism.[7]

In Europe, the Swedish forest products company Svenska Cellulosa Aktiebolaget uses a triquetra valknut as their logo, which can be commonly seen on many products produced by the company; the DFB has used a logo inspired by the unicursal form of the valknut for the Germany national football team since 1991.[8]

The symbol appears as the fretboard inlay on some of Arch Enemy/Carcass guitarist Michael Amott's signature Dean Guitars "Tyrant" models, and it is also used as a logo by American engineering firm RedViking.[9] In Civilization VI, the valknut is the national symbol of Norway, which in the game is led by Harald Hardrada and mostly representative of Vikings rather than the modern country.

The valknut has seen some use by White supremacists.[10][11] The Anti-Defamation League notes that "nonracist pagans may also use this symbol, so one should carefully examine it in context rather than assume that a particular use of the symbol is racist."[10]

Skulls aren't exactly unique on unit patches and the like.

Some kind of individual patch at the whim of the owner. Maybe just a trophy.
Such patches are offered in the Ukrainian military store in Kyiv:

As for the combined triangles, this is one of the symbols of the religious culture of the pagans:

Pagan symbols are also sold there:

This guy seems to have stocked up at the Kiev store ;-)

The shop sells the skulls as: R3ICH НАШИВКА OPERATOR SKULL
i.e. R3ICH.de
Also available on eBay.
I saw one suggestion that the key resembles the 1st SS Div insignia.

Yes, if you look closely, then on the helmet near the skull you can see an image of an emblem similar to the emblem of the 1st SS regiment.

Yes, I remember the UK's 17th Lancers' Insignia from the WWII NA Campaign. IIRC they were an armored recon unit with Armored Cars. old fart

Skull insignia is popular with many in the military, or even bikers, etc. In most cases, the overwhelming majority, they have nothing to do with the Nazis or the SS. E.g. in the 101, our Co T-shirt had a skull on it. Nothing to do with Nazis. IIRC as a Mech Co. Cdr my CVC had a skull with fangs drawn on it. Can't remember if I or one of my "artistic" troopers did it ? old fart Again … nothing to do with Nazis/SS …

The "Punisher" Skull IIRC from the Marvel Comic is particularly favored by US Forces … E.g. link

Skull insignia for units date back to at least medieval times.

The 72nd Mechanized Brigade (Ukraine) uses a jaw-less skull on its sleeve patch:

Supposedly of Cossack origin.

If they include more than one skull they'll get sued by GW… ;-)
