Pandemic. I fly international 12 – 20 times a year and, without fail, pick up some sort of bronchial infection. I can't be the only person with this lack of resistance so – I bet on pandemic carried by pressurised tin cans at 10kM where 200+ people breath each others recycled air.

It was a hostile world, but the mining companies paid a lot for people to work there. Then one day, the galactic stock market crashed and all these people found that the supply ships just weren't coming around any more. And then it turned out there were natives living in the damn place too, not to mention the mutants and bioweapon research projects that weren't being contained anymore…

A saurian race of aliens, the Garn, attempt to change the Earth's climate back to Mesozoic conditions using giant stealth terraformers that hover at various points in Earth's atmosphere. The resulting rise in global temperatures melts the ice covering on the poles and releasing the long dormant spores of a variety of Cordyceps fungus that is deadly to hominids. Carried along by the jet stream, the fungal spores infect millions across the globe, turning them into fungus zombies that are compelled to attack the uninfected in turn, turning them into fungus zombies. Unable to discover the mysterious cause of the pandemic and climate change, the US, Russia, and China blame each other and launch nuclear strikes, but these are limited due to so many falling to the Cordyceps Zeds. As the story opens, small bands of human survivors face the return of the hungry Garn.

What, no mention in the poll of:

That cult would never die till the stars came right again, and the secret priests would take great Cthulhu from His tomb to revive His subjects and resume His rule of earth. The time would be easy to know, for then mankind would have become as the Great Old Ones; free and wild and beyond good and evil, with laws and morals thrown aside and all men shouting and killing and revelling in joy. Then the liberated Old Ones would teach them new ways to shout and kill and revel and enjoy themselves, and all the earth would flame with a holocaust of ecstasy and freedom.
