Are those horns on his head? I know Milagroso means Miraculous, but I have to wonder where this miracle is really coming from!

Swept hair, teased to make you think of horns, arched eyebrows, a small animal skull on the chest, dental pliers in one hand, a potion – opium perhaps – in the other and the shoes! Dead classy shoes.
