Any good books that discuss this or even a decent online resource?

I've poked around the internet a bit but it seems the system was employed in all manner of ways.

Some sources seem to indicate it was mainly officers, NCO's and specialists while others go as far down as machine gunners and the occasional runner or rifleman.

Any clarification or pointers?

You have to dig in to regimental histories to really see this in operation. It varied, but by Sicily in 43 it was pretty standerd to keep a cadre around which the previous 4 years of training could be passed on an rapidly integrated into the front companies (at in British & Commonwealth forces long-trained in the UK.

I haven't delved deep enough to see the guidance orders, and of course each regiments OR would implement the hold-back as they saw fit, within the parameters.

I know from accounts a lot of platoon/company officers made note in letters when LOB worked in their favour.
