There is apparently a paper miniature of one included in this pdf product from Paizo.

As I understand it this creature is a sort of vine plant thing so it should be easy to craft one from several printings of the paper one, cut out and glued together to give a 3D effect.

It is more an area effect than a true monster, so I would probably go with a large base with green vines made of green string flocked with ground foam and a few yellow flowers made from either ground foam or tiny bits of yellow paper.

How about using etched brass foliage, perhaps emerging from a skull on the base to emphasise the point.
