When activating a magic user, do you always have to declare his intention to cast a spell? Or is that only if he's rolling 3 dice (due to the potential of losing his power)?

I believe it is only if rolling 3 activations.

only if rolling 3 dice. Different magic using types, shown in the supplements, might have different rules for triple failures.
