Fritz Leiber's Nehwon/Lankhmar is my favorite fantasy setting. Most D&D editions have had a sourcebook for this most excellent fantasy world.

In no particular order, the Warhammer setting, China Miéville's Bas-Lag (the setting of Perdido Street Station) and as Alex says, Crocodile Games' setting.

All rather "high-fantasy" magic-soaked settings, the way I like 'em. I want my fantasy setting to be fantastic, not just the middle ages with extra apostrophes and some funny-looking foreigners.

Hyboria, it has a pre history period with ancien ruins and magic that linger on into the "current" gener setting. It has detail throughout the world, epic battles for power, small battles along borders and indiviuals adventuring. With several authors having filled out the story line along with Comic Book editions it has a wide following, many people know what it is without long explanation. Magic can or is not always in every setting which provides a great variety of possible game settings. Armor and weapons span bronze age thru to medieval.
